Senior Practitioner – Children with Disabilities Team

I am looking for an experienced Senior Practitioner to work with children and young people with significant and profound, permanent disabilities in Chessington.
In this role you will be:
- Working directly with children who are subject to a Child Protection Plan, or who have Child in Need status or who is a Looked After Child
- Assessing the needs of children with disabilities and their families
- Developing individualised plans, and chairing meetings to ensure plans are on track in partnership with parents/carers and other professionals involved in the child's life
- Using a range of interventions and skills to engage children, young people and their families in working towards the objectives of their individual care plans and ensuring that the voice of the child is heard
- Developing and implementing projects as supported by Head of Service
- Contributing to the supporting of students on placement, and newly qualified staff (some opportunity to complete Practice Educators Course/other relevant training)
- Acting as a link person for one of our local specialist schools
For this post you must have:
- A Social Work qualification and registration with Social Work England
- Experience in working with Children and young people who have profound and severe disabilities
- Experience of building strong partnerships, maintaining relationships and co-producing solutions to challenges across a diverse set of stakeholders, including children, young people and their families
- Experience of keeping and maintaining records (in accordance with local policy) and producing accurate reports, presentations, written updates to a range of stakeholders
- Knowledge of statutory legislation (for example Children’s Act) and national policy drivers that will affect children’s services and your role (in this role it will include knowledge of and understanding of the KS2 and KS3 national curriculum)
- Experience of addressing specific equality or diversity issues within your practice with colleagues/service users and taking action to solve them
